The Community Health Worker Program is a community-based, family-centered program with the goal of achieving optimal health status among Buffalo's pregnant and parenting women. Community Health Workers (CHWs) reach out to families in "high-risk" areas having high infant mortality and poor socioeconomic status. This program will provide home-based case management services, assess women and children that have been recruited and provide assistance in overcoming barriers to care.

The Community Health Worker's will:

  • Identify complicating factors that make it difficult for clients to access the care they need.
  • Assist in enrolling pregnant women into prenatal care and children into primary care.
  • Assist in enrolling for services such as WIC, Medicaid/PCAP and Child Health Plus.
  • Provide education on topics such as breastfeeding, baby care, lead poisoning, immunization, nutrition, HIV/AIDS, domestic violence, and substance abuse.
  • Provide referrals to food pantries, housing assistance, educational programs, childcare, and other services needed.
  • Most importantly provide support and help to overcome the barriers that prevent families from receiving a healthy start in life.
These services are offered at no cost, to eligible consumers residing in the following zip codes: 14201, 14204, 14207, 14208, 14209, 14210, 14211, 14212, 14213, 14214, 14215. All services are available in English and Spanish.
